When he says "Oh my god" (notice the lower-case g) it is him saying it in vain and anger. He is not saying "God, I want to tear you apart", he is saying "Oh my god" in frustration and then says "I want to tear you apart" to the real person he is angry at, probably the one with "cancer lips". Hope that clears things up.
